Pharmacology & Mechanism

Actovegin is a highly purified ultrafiltrate of calf blood. It enhances cellular oxygen and glucose uptake, promoting ATP synthesis under hypoxic conditions. It also modulates NF‑κB signaling, inhibits PARP activity, and reduces apoptosis. Vascular effects include vasodilation via nitric oxide synthase, reduced arteriolar tone, and improved red blood cell deformability.

Onset of action: ~30 minutes. Peak effect: 2–6 hours after oral administration.

Indications

  • Post-stroke cognitive impairment (PSCI) and vascular dementia
  • Peripheral arterial or venous circulatory disorders and trophic complications
  • Diabetic peripheral neuropathy (DPN)

Contraindications

  • Hypersensitivity to components
  • Fructose intolerance, glucose-galactose malabsorption, sucrase-isomaltase deficiency
  • Children under 18 years

Use in sports:

Actovegin is widely used among athletes for its metabolic-enhancing and antihypoxic properties. Clinical data indicate that Actovegin improves oxygen uptake and glucose utilization at the cellular level, especially in skeletal muscles and the brain. This makes it potentially valuable during intense physical exertion or in hypoxic conditions. Athletes report faster recovery times, reduced muscle fatigue, and improved endurance, particularly during periods of high-intensity training or competition.

Moreover, Actovegin may support microcirculation and capillary perfusion, which enhances nutrient delivery and waste removal in working muscles. Some studies suggest it can accelerate muscle repair and tissue regeneration, making it useful in injury recovery protocols. While the drug is not classified as a performance enhancer by WADA, its use is often viewed as part of legal recovery and optimization strategies in high-performance sports.

Use During Pregnancy & Lactation

Limited data. Use only if potential benefit justifies potential risk to fetus or infant.

Dosage & Administration

Swallow whole before meals with water.

  • PSCI: IV 2000 mg/day (≤ 20 infusions), then oral 3×2 tablets/day (1200 mg/day) for 6 months
  • Dementia: Oral 3×2 tablets/day for 20 weeks
  • Circulatory disorders: 1–2 tablets, 3×/day, for 4–6 weeks
  • DPN: IV 2000 mg/day × 20 infusions, then oral 3×3 tablets/day (1800 mg/day) for 4–5 months
